Transitional Care

Our main goal is to help patients achieve the highest level of self-sufficiency and independence before returning home.

 

The healing process is different for everyone. Sometimes the road to recovery involves a detour through rehabilitation or additional therapy, especially after surgery or in medically complex cases. Signature Healthcare Brockton Hospital’s Transitional Care Unit (TCU) is a 29-bed facility located within the hospital that provides skilled nursing and rehabilitation services to adults with complex medical, surgical, and orthopedic needs in a home-like environment.

The TCU was recently found to be in substantial compliance with The Department of Public Health-Long Term Care Division requirements, a testament to the hard work of the staff. These reviews occur every 9-15 months. In addition, the unit conducts "On The Spot" satisfaction surveys with all patients using iPad technology during their stay, allowing immediate feedback to administrators and prompt resolution of any issues.

From the moment you enter, you'll notice a different kind of atmosphere. We do everything possible to make you feel more at home and less like a patient. 

We have a highly trained group of therapists on the TCU floor with their own gym, so you can receive all your therapy without traveling to a separate facility.

 

 We'll  encourage you to participate in as many daily living and fun recreational activities as you can.
